style=\"font-weight: 400;\">The Regional Knowledge Event is a regular meeting organized by ANHSS, serving as a discussion forum for policymakers, legislators, practitioners, industry players, and academics across the region. This May 2025 event was attended by over 120 participants from eight countries in Asia.<\/span><\/p>\n<h3><b>Opening Session<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Opening remarks were delivered by Professor Philip Wai-Yan Chiu, Dean of the Faculty of Medicine at The Chinese University of Hong Kong; Professor Samuel Yeung-Shan Wong, Director of the JC School of Public Health and Primary Care at the same university; and Professor Dr. Laksono Trisnantoro from Universitas Gadjah Mada, Indonesia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The speakers highlighted that Universal Health Coverage (UHC) principles underpin efforts to integrate resources between the public and private sectors. The private sector was recognized as a crucial partner to government efforts, serving as both a <\/span><b>supplementary<\/b><span style=\"font-weight: 400;\"> and <\/span><b>complementary<\/b><span style=\"font-weight: 400;\"> mechanism to public systems. He described four different pooling models:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>No pooling<\/b><span style=\"font-weight: 400;\">, where individuals pay out-of-pocket;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Unitary pooling<\/b><span style=\"font-weight: 400;\">, where all funds and risks are combined in one national pool (e.g., Indonesia);<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Fragmented pooling<\/b><span style=\"font-weight: 400;\">, where risk is separated by geography, risk group, or population (e.g., Germany, the UK, Thailand); and<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Integrated pooling<\/b><span style=\"font-weight: 400;\">, where funding can shift across pools in exceptional circumstances.<\/span><span style=\"font-weight: 400;\"><br \/><br \/><\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">He reiterated the importance of PHI as a supplementary and complementary solution to public financing schemes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">From a regulatory perspective, Mr. Clement Lau Chung-Kin of the Insurance Authority (Hong Kong) stated that the government holds a dual mandate: to <\/span><b>protect the public<\/b><span style=\"font-weight: 400;\"> while also ensuring a <\/span><b>sustainable insurance industry<\/b><span style=\"font-weight: 400;\">. Insurance, he emphasized, also shapes societal behavior and individual decision-making.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mr. Sam Hui, JP, Deputy Secretary for Health, explained that only <\/span><b>30% of private healthcare financing in Hong Kong<\/b><span style=\"font-weight: 400;\"> is covered by PHI, with <\/span><b>60% still paid out-of-pocket (OOP)<\/b><span style=\"font-weight: 400;\">. He noted that there is a <\/span><b>significant untapped market<\/b><span style=\"font-weight: 400;\"> for PHI, but also outlined strict requirements imposed by regulation:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Plans must be <\/span><b>renewable up to age 100<\/b><b><br \/><\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">There must be <\/span><b>no lifetime claim cap<\/b><b><br \/><\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Pre-existing conditions must be covered<\/b><span style=\"font-weight: 400;\"> starting from the second year<\/span><span style=\"font-weight: 400;\"><br \/><\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Benefits must <\/span><b>expand progressively from year four<\/b><b><br \/><\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">In addition, the government demands <\/span><b>price transparency<\/b><span style=\"font-weight: 400;\"> and efforts to reduce <\/span><b>information asymmetry<\/b><span style=\"font-weight: 400;\">. However, consolidated PHI data remains limited, prompting the government to initiate regular dialogues with the industry.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Eduardo Banzon from the Asian Development Bank reminded participants that the \u201cuniversal\u201d in UHC means <\/span><b>everyone should have access<\/b><span style=\"font-weight: 400;\"> to the health services they need. Across Asia, many countries are shifting service provision to the private sector, except where private actors are deliberately restricted. He noted that in countries where population coverage under UHC is not yet achieved, PHI might act as a <\/span><b>substitute<\/b><span style=\"font-weight: 400;\">. However, this is not ideal from a public finance perspective, as it could erode revenue from national insurance pools.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Instead, PHI should function as a <\/span><b>complementary<\/b><span style=\"font-weight: 400;\"> and <\/span><b>supplementary<\/b><span style=\"font-weight: 400;\"> solution\u2014expanding service coverage and improving financial protection. He shared that in the <\/span><b>Philippines<\/b><span style=\"font-weight: 400;\">, voluntary PHI is <\/span><b>non-taxable<\/b><span style=\"font-weight: 400;\"> and only available to <\/span><b>PhilHealth members<\/b><span style=\"font-weight: 400;\">, making it attractive to the <\/span><b>middle- and lower-middle-income groups<\/b><span style=\"font-weight: 400;\">. Indonesia, he added, once implemented <\/span><b>Coordination of Benefits (CoB)<\/b><span style=\"font-weight: 400;\">, but the policy was paused in 2020.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mr. Alger Fung, CEO of AIA Hong Kong and Macau, provided the <\/span><b>industry perspective<\/b><span style=\"font-weight: 400;\">, emphasizing that PHI not only offers health protection but also contributes to <\/span><b>public health promotion<\/b><span style=\"font-weight: 400;\">. AIA, for example, provides <\/span><b>premium discounts<\/b><span style=\"font-weight: 400;\"> to customers who improve their health metrics, creates <\/span><b>public parks<\/b><span style=\"font-weight: 400;\">, and runs <\/span><b>free wellness programs<\/b><span style=\"font-weight: 400;\"> like yoga, boxing, and tennis classes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Professor Jeremy Lim from Singapore described a <\/span><b>multi-tiered insurance system<\/b><span style=\"font-weight: 400;\"> built to serve both the general population and high-income or expatriate communities. Panelists emphasized the importance of <\/span><b>strong regulations<\/b><span style=\"font-weight: 400;\">, <\/span><b>governance capacity<\/b><span style=\"font-weight: 400;\">, and <\/span><b>data infrastructure<\/b><span style=\"font-weight: 400;\"> to support PHI\u2019s contribution to national health systems. Transparent, accountable systems are essential to allow <\/span><b>data sharing<\/b><span style=\"font-weight: 400;\"> for <\/span><b>policy development<\/b><span style=\"font-weight: 400;\"> and <\/span><b>monitoring<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3><b>Afternoon Session: Industry and Country Experiences<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">In the afternoon keynote, <\/span><b>Professor Soonman Kwon<\/b><span style=\"font-weight: 400;\"> from <\/span><b>Seoul National University<\/b><span style=\"font-weight: 400;\"> discussed how PHI operates under South Korea\u2019s universal public insurance system. In Korea, PHI acts as a \u201c<\/span><b>fiscal valve<\/b><span style=\"font-weight: 400;\">\u201d for the public scheme by covering <\/span><b>co-payments<\/b><span style=\"font-weight: 400;\"> and services not included in the national plan. This results in <\/span><b>increased use<\/b><span style=\"font-weight: 400;\"> of both private and publicly reimbursed services.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Healthcare providers in Korea often engage in <\/span><b>bundling<\/b><span style=\"font-weight: 400;\"> of services that are covered and not covered by public insurance, which contributes to higher overall utilization.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. <\/span><b>Yat Chow<\/b><span style=\"font-weight: 400;\"> from <\/span><b>BUPA<\/b><span style=\"font-weight: 400;\"> noted that in <\/span><b>Hong Kong<\/b><span style=\"font-weight: 400;\">, PHI is most frequently used for <\/span><b>outpatient services<\/b><span style=\"font-weight: 400;\">, especially in <\/span><b>primary care<\/b><span style=\"font-weight: 400;\">, including both <\/span><b>Traditional Chinese Medicine (TCM)<\/b><span style=\"font-weight: 400;\"> and <\/span><b>Western Medicine<\/b><span style=\"font-weight: 400;\">. Out-of-pocket payments still dominate, although Hong Kong\u2019s health system reflects a relatively balanced <\/span><b>public\u2013private financing blend<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For example, <\/span><b>HK$29 billion<\/b><span style=\"font-weight: 400;\"> in outpatient care is financed by individuals, while <\/span><b>HK$6.8 billion<\/b><span style=\"font-weight: 400;\"> comes from the government. In fiscal year 2023, <\/span><b>public health expenditure<\/b><span style=\"font-weight: 400;\"> reached <\/span><b>HK$150.4 billion (58%)<\/b><span style=\"font-weight: 400;\">, and <\/span><b>private expenditure<\/b><span style=\"font-weight: 400;\"> stood at <\/span><b>HK$109.7 billion (42%)<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3><b>Country Experiences: Thailand, Malaysia, and Indonesia<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">The session continued with presentations from Thailand, Malaysia, and Indonesia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In <\/span><b>Thailand<\/b><span style=\"font-weight: 400;\">, Professor Chantal Herberholz explained that PHI generally takes two forms: <\/span><b>individual plans<\/b><span style=\"font-weight: 400;\"> for high-income groups and <\/span><b>group plans<\/b><span style=\"font-weight: 400;\"> tied to formal employment or companies. Claims are disproportionately high in major urban centers with advanced infrastructure, creating disparities in access. At the same time, consistently high <\/span><b>claim ratios<\/b><span style=\"font-weight: 400;\"> strain the public insurance system\u2019s financial sustainability. Claims fell during the COVID-19 pandemic due to reduced service utilization but have since risen again, contributing to a widening deficit.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In <\/span><b>Malaysia<\/b><span style=\"font-weight: 400;\">, <\/span><b>Professor Sharifa<\/b><span style=\"font-weight: 400;\"> described the rapid growth of private healthcare and PHI, supported by strong demand and the country\u2019s <\/span><b>medical tourism sector<\/b><span style=\"font-weight: 400;\">. PHI is regulated by the <\/span><b>Central Bank of Malaysia<\/b><span style=\"font-weight: 400;\">, ensuring financial oversight. However, <\/span><b>medical inflation<\/b><span style=\"font-weight: 400;\"> and increasing <\/span><b>utilization<\/b><span style=\"font-weight: 400;\"> have driven PHI premiums upward. Projections indicate that premiums may rise by <\/span><b>40% to 70% by 2025<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><b>Conclusion<\/b><\/p>\n<p><span style=\"font-weight: 400;\">This regional knowledge exchange revealed both the diversity and similarity in approaches to private health insurance across Asia.
