The event was organized by the <strong>Asia-Pacific Network for Health Systems Strengthening (ANHSS)<\/strong> in collaboration with the <strong>Centre for Health Systems and Policy Research at the Jockey Club School of Public Health and Primary Care, The Chinese University of Hong Kong.<\/strong><\/p>\n<p>In his opening remarks, Prof. Laksono highlighted that the role of private health insurance within the pursuit of Universal Health Coverage (UHC) remains a subject of considerable debate among health sector stakeholders.<\/p>\n<p>\u201c<em>Within the health community, the role of private health insurance in achieving UHC continues to generate both support and criticism. However, maintaining both quality and accessibility of healthcare services remains a significant challenge. At present, healthcare financing continues to rely heavily on government funding.<\/p>\n<p>At the same time, Hong Kong is facing a rapidly ageing population, a growing burden of chronic diseases, and escalating healthcare costs. These developments have substantially increased the demand for healthcare financing, making it increasingly difficult to depend solely on public resources. For decades, the government has borne a considerable financial burden, reflected in part by lengthy waiting times within the public healthcare system.<\/p>\n<p>Consequently, there is a growing need to identify additional financing sources so that government resources can be more effectively targeted as a safety net for vulnerable populations. In this context, private financing plays an increasingly important role in creating a more balanced and sustainable healthcare financing system.<\/p>\n<p>A key challenge moving forward is how to increase private sector contributions in line with the principle of \u201cYour Life, Your Health,\u201d which emphasizes individual responsibility for healthcare financing. To achieve this objective, the utilization of private health insurance must continue to expand. Insurance products should be designed with appropriate pricing strategies, while coverage arrangements should be clearly defined to meet the evolving needs of the population.<\/p>\n<p>Nevertheless, no single financing model can serve as a universal solution. Several fundamental principles must be incorporated, including strengthening primary healthcare systems, implementing proportionate co-payment mechanisms, protecting low-income populations and persons with disabilities through safety-net arrangements, and supporting the financing of primary care services. Furthermore, clinical quality must be safeguarded through effective governance and robust quality assurance systems. Through the combination of these approaches, healthcare financing systems can become more sustainable, equitable, and capable of delivering high-quality care.<\/p>\n<p><span style=\"font-size: 14pt;\"><strong>Dr. Eduardo Banzon<\/strong><\/span><\/p>\n<p>Director, Health Practice Team, Asian Development Bank (ADB), Manila<\/p>\n<p><img decoding=\"async\" class=\"alignleft\" src=\"https:\/\/kebijakankesehatanindonesia.net\/wp-content\/uploads\/2026\/06\/Hk-3-225x300.jpeg\" \/>As Director of the <strong>Health Practice Team at the Asian Development Bank (ADB)<\/strong>, Dr. Eduardo Banzon presented an overview of private health insurance development across Asian countries. His analysis examined the relationship between out-of-pocket healthcare expenditure and levels of effective health coverage. He also reviewed trends in private health insurance development across Asia using data from <strong>National Health Accounts.<\/strong><\/p>\n<p>In South Asia, the development of private health insurance remains relatively limited, with India being a notable exception. Bangladesh, meanwhile, continues to rely heavily on out-of-pocket spending as a primary source of healthcare financing.<\/p>\n<p>In Southeast Asia, the development of private health insurance in Indonesia and the Philippines has not yet reached its full potential. One contributing factor is the absence of strong coordination mechanisms between social health insurance schemes and private health insurance providers. This challenge is evident in the relationships between PhilHealth and private insurers in the Philippines, as well as between BPJS Kesehatan and private insurers in Indonesia.<\/p>\n<p>In contrast, Singapore has demonstrated stronger integration between public and private financing mechanisms, while Thailand has achieved a relatively higher level of private health insurance utilization. Vietnam has also emerged as an interesting case, having experienced significant growth in private health insurance development compared to previous years.<\/p>\n<p>In East Asia, South Korea has increasingly expanded opportunities for private health insurance to play a larger role within its healthcare system. Similarly, several European countries, including the Netherlands and Switzerland, have long promoted private health insurance as an integral component of national healthcare financing systems.<\/p>\n<p><strong>Dr. Banzon emphasized that approaches to private health insurance development vary considerably across countries and regions. Persistently high levels of out-of-pocket healthcare expenditure indicate that social health insurance alone cannot achieve Universal Health Coverage (UHC)<\/strong>. Effective policies are needed to integrate and complement social health insurance with private health insurance arrangements.<\/p>\n<p>With appropriate regulation, private health insurance can function as both a complementary and supplementary source of healthcare financing alongside government-funded schemes. This system is complemented by high-quality services delivered by private healthcare institutions and practitioners. Currently, approximately 51% of the population is covered by either individual or group health insurance schemes.<\/p>\n<p>However, the dual-track healthcare system is increasingly challenged by population ageing, workforce shortages, and rapid technological advancements. Despite various initiatives to address these challenges, including the introduction of the Voluntary Health Insurance Scheme (VHIS) in 2019, household healthcare expenditure remains relatively high at 32%, while government subsidies continue to account for approximately 52% of total healthcare financing.<\/p>\n<p>In response, the Insurance Authority has initiated a comprehensive review of the health insurance sector aimed at narrowing protection gaps through enhanced data analytics and evidence-based product design. The Authority also seeks to strengthen regulatory oversight to ensure that consumers can make informed decisions and receive fair treatment, while enabling the insurance industry to manage costs effectively and maintain long-term sustainability.<\/p>\n<p>In addition, the government implemented reforms to healthcare fees and charges beginning on 1 January 2026 and established the Institute for <em>Medical Advancement and Clinical Excellence<\/em> (IMACE) to support the development of clinical protocols and promote healthcare quality improvement.<\/p>\n<p>\u00a0<\/p>\n<h3>Role of Private Health Insurance in Healthcare Financing to Advance Universal Health Coverage<\/h3>\n<p><span style=\"font-size: 14pt;\">Speaker: <strong>Professor Eng-kiong Yeoh, GBS, OBE, JP<\/strong><\/span><\/p>\n<p>Director, Centre for Health Systems and Policy Research<br \/>JC School of Public Health and Primary Care<br \/>The Chinese University of Hong Kong<\/p>\n<p><img decoding=\"async\" class=\"alignleft\" src=\"https:\/\/kebijakankesehatanindonesia.net\/wp-content\/uploads\/2026\/06\/HK-4-yeoh-225x300.jpeg\" \/>As a risk-transfer mechanism, Private Health Insurance (PHI) pools risks and financial contributions from its members and redistributes resources to protect individuals against unexpected health events and catastrophic healthcare expenditures. PHI also provides consumers with greater choice in accessing healthcare services.<\/p>\n<p>Professor Yeoh presented findings from a literature review examining the role of private health insurance in supporting Universal Health Coverage (UHC). The review found limited consensus regarding the effectiveness of PHI as a primary insurer, duplicate insurer, or supplementary insurer within healthcare systems. Most evidence suggests that PHI primarily serves a supplementary role alongside publicly financed health coverage.<\/p>\n<p>The findings indicate that robust regulatory frameworks and institutional strengthening are necessary to ensure that PHI contributes constructively to UHC objectives. Several countries have adopted mandatory PHI arrangements, such as the Netherlands and Switzerland, while others have promoted PHI through tax incentives or penalty-based mechanisms, as seen in Hong Kong and Australia.<\/p>\n<p>Professor Yeoh concluded that the contribution of PHI to UHC depends largely on policy design, regulatory oversight, and its ability to complement existing public financing arrangements.
